Monogram Health Publishes Inaugural Impact Report, Documenting Impressive Clinical and Financial Outcomes Resulting From Its In-Home Multispecialty Medical Group Care Model

The report represents Monogram's first formal public summary of the clinical, financial, and social impact its care model delivers to patients, health plan partners, and the broader healthcare system.

As value-based care continues to attract scrutiny regarding its real-world performance, Monogram's report reveals measurable outcomes — providing payers, providers, policymakers, and the public with a data-driven view of what in-home multispecialty treatment can achieve for the country's most clinically complex patients.

Patient Impact & Health Outcomes

Central to the report is a detailed account of how Monogram's care model drives tangible health improvements for the patients it serves — individuals who, before Monogram, were routinely cycling through emergency departments, skilled nursing facilities, and fragmented specialist networks without a coordinated treatment plan.

Monogram's 2025 results reflect the depth of its multispecialty care model. Across its attributed patient population of more than 220,000 individuals — 94% of whom carry four or more chronic conditions — Monogram clinicians delivered 435,000+ patient visits and 150,000 clinical interventions within the year. The company reports that 87% of patients achieved a controlled A1C, 80% achieved controlled blood pressure, and 1,389 total years of life were saved.

The quality outcomes documented in the report speak to the effectiveness of its multispecialty model. For patients with congestive heart failure, Monogram was able to stabilize its patients' conditions to the extent where flareups requiring hospitalization were reduced by 22% and rehospitalizations for some exacerbated conditions were reduced by 35%. For patients with diabetes, those figures reached 29% and 48%, respectively. Among the most significant improvements documented across any disease state in the report was patients with COPD, who saw a 25% reduction in the need for hospitalization from a flare-up of their pulmonary condition(s).

Closing Gaps in Access and Social Determinants of Health

The report highlights how Monogram's care model extends well beyond clinical treatment. Approximately 25% of Monogram's patients reside in rural communities, and 77% live in counties below the poverty line. In 2025, Monogram's licensed clinical social workers closed more than 35,000 Social Determinants of Health gaps, connecting patients to food resources, transportation, pharmacy assistance, financial support, and other community benefits. Monogram Health FIRST, the company's 24/7 at-home care program, handled 18,000 inbound patient consultations and averted more than 7,700 unnecessary hospital and emergency department visits.

The report also details Monogram's Comprehensive Medication Review program, which completed 88,900 medication reviews in 2025, prescribed and optimized 35,000 medications, resolving more than 92% of high-priority drug recommendations in alignment with clinical guidelines — a critical safeguard for a patient population averaging 12 or more medications per day across five or more prescribers.

Patient Experience and Financial Performance

Across its patient population, Monogram achieved a Patient Net Promoter Score of 72 and a 1.15 Stars improvement, reflecting a care experience that is not only clinically effective but consistently well-regarded by those receiving it.

In 2025 alone, Monogram delivered $342 million in total medical savings, achieved a 7.4% reduction in medical loss ratio across its attributed population, and managed $4.9 billion in care spend. Across the diseases it treats — congestive heart failure, diabetes, cardiovascular disease, COPD, and end-stage kidney disease — Monogram improved patients' outcomes and reduced admissions-per-thousand ranging from 22% to 29%, and reduced readmissions-per-thousand ranging from 35% to 48%, all well above national benchmarks.
